Goal Evidence Engine
Evidence from your own words.
Say it or type it as it happens.
One tap turns your shift into evidence.
Built for NDIS support workers. Speak or type short notes during the shift. When you're done, one tap writes a clinical report from what you recorded, ready to paste into ShiftCare.
During the shift
- 3:12 pmRose chose the pool today. She swam two laps then had a rest.
- 4:45 pmShe ordered her own juice at the kiosk and paid with her card.
- 6:02 pmCalm drive home. She wants to swim again on Friday.
The report
Rose attended the pool and swam two laps before taking a rest.
At the kiosk she ordered her own juice and paid with her card.
Anything to hand over for the next shift?
Underlined words are the worker's own. The amber question is a gap the AI asks about instead of filling in.
How a shift becomes a report
Capture as it happens
Speak or type short entries through the shift. Each one is stamped with the time and saved on the spot. No signal? Entries wait on your phone and sync when you're back. No AI touches the capture screen.
One tap when you're done
Tap Generate shift report. The AI writes the full shift, arrival to handover, with times woven in. Every factual sentence must trace to your recorded words. Anything it can't back becomes a question for you, not a sentence in the report.
Copy into ShiftCare
One button puts the whole report on your clipboard as plain text, with your original entries underneath. Copying is never blocked. An open question just means a shorter, fully checked paste.
It prompts and it proves.
Every factual claim in a generated report must cite a fragment that really appears in your entries, checked in code before the sentence can leave the app. A sentence that fails the check turns amber and stays out of the paste. Gaps get a question, never filler. Medication, incidents, injuries: if your entries don't mention them, the report won't invent them. It asks you. Your answer lands as a new timestamped entry and the report regenerates.
The check proves the words are really there. Whether they show real progress toward a goal stays a human call. That's what review is for.
For coordinators
Each participant goal builds a timeline from adopted reports only. Before an audit, pick a participant and a date range, then print the audit pack in one tap: every verified quote, dated and attributed to the worker who recorded it. Audit-ready evidence, aliases only.
Colour is provenance
- AI proposed
- Human confirmed
- Needs attention
- Milestone
Lavender is AI-proposed and waits for a person. Emerald means a person confirmed it. Amber flags what needs attention first.
